Do I have to have an e-mail account to request an online consultation?

Yes. The For Dynamic Living staff will need to communicate with you via email if you request an online consultation; therefore, an email account is required. Several Web sites offer free email accounts, including Yahoo!, iVillage, Hotmail and Juno.

 

Can I request a For Dynamic Living online consultation for someone else?

Yes, if you have the client's permission. If the client is 18 years of age or older, he or she must be capable of providing written consent. Print the For Dynamic Living Consult Proxy Form and have the client complete, sign and date the form, and email the form to Form Submission We cannot proceed with the assessment requested by one individual for another without a proxy form signed by the client for whom the consultation will be conducted.

 

Can I send my medical records to For Dynamic Living in a language other than English?


We are not able to process requests that are not in English and we do not provide a document translation service. All submissions pertaining to your online consultation must be translated prior to being submitted to the Consult office. Medical translation services (not within For Dynamic Living) are available and we would suggest researching to find the one that best suits your needs.

Can I speak with or email the wellness advocate who authored my online consultation?



When your consultation is ready, you will be notified via email. In order to ensure that you understand the content of the consultation and your questions have been answered, you will have a follow-up appointment scheduled with the wellness advocate. Or you may email any questions and we will respond to your inquiry. In some cases, the wellness advocate may wish to speak with you, at which time our office will facilitate the call.

When should I expect to receive my online consultation assessment?
The timeframe for rendering your online consultation is typically 10-14 business days. However, the duration of time does not begin until our wellness advocates have processed through all of the materials they have received and determined that they have all of the necessary medical information from the client and an appropriate wellness advocate is available, and assigned the case. Our goal is to deliver a comprehensive consultation to you so you can make an informed decision about your health or the health of a loved one. In the event there will be any delay in rendering your online consultation, you will be contacted by the For Dynamic Living office.


Will you store my credit card information?
When you request an online consultation and provide your credit card information to us, it is processed thru PayPal processing system, and we do not store the information in our system. If you request an additional online wellness consultation at a future date, we will ask you to provide your credit card information again. See our Privacy Statement for a full explanation of our information and security practices.

Can I cancel my online consultation request?
You may cancel your online consultation request at any time while you are filling out the online forms. However, you may not cancel your consultation once you have authorized us to proceed (upon completion of the online steps).
